Why are antibiotics prescribed in the US?

I have heard that it is much easier to get antibiotics in Italy and China. Some have told me that they were able to get these medicines without a written prescription.
Given that Italy is a western county and China is doing so well they lend the United States the money that is keeping us afloat, it would seem that having some medicines freely available does not cause the direct and immediate collapse of a country.
Why are the same medicines that require a prescription in the US freely available in other nations? Are their hidden variables that are keeping costs high and health care quality low in the US? Are the real cost savings in an area that is not even being talked about in the US? For instance, suppose we all stopped buying medicines in the US altogether and bought all our drugs from Canada. Wouldn't that bring our costs down?
What is the effect of having antibiotics freely available over the counter in non US countries? Are they contributing to the creation of super drug resistant bugs? Are they making the drugs burn out sooner? By burning out I mean that the over use of antibiotics tends to make them less effective.
